for 3d visuals I think morphyre (http://www.morphyre.com)is the best choice,
but to be honest, I've returned to milkdrop as the visuals feel more "alive" :)
and some nice little things are not...

have you tried to start winamp as Administrator...
have you tried to start winamp as Administrator and do the config change?
if you use Win7/Vista and Installed Winamp under C:\Program Files or C:\Program Files(x86)
it could be a permission problem...
