I have to say Silent Hill has made some of the scariest sounds I've ever encountered. Doom 3 is pretty close too.

I think the most scariest game I've ever played though is FEAR. For me. The scenes and lights flickering, just the atmosphere is crazy and keeps me on the edge of my seat.
