On my XP PC, I have just looked at EXACTLY what is ticked.

Preferences \ General Preferences \ File Types
TICKED - Enqueue files on double click in Windows Explorer
UNTICKED - Show Winamp in the folder context menus in Windows Explorer

Preferences \ General Preferences \ File Types \ Shell Options
TICKED - Add Enqueue & Play to Winamp's associated file types
TICKED - Set Enqueue & Play as the default file action
HIGHLIGHTED - Play in Winamp
HIGHLIGHTED - Enqueue and Play in Winamp

When I double click on an MP3, it is added to the playlist, and immediately starts playing that track.

If I don't tick "Set Enqueue & Play as the default file action" this leaves the default to another application on the PC.

Are you 100% SURE no other media player program is nicking these associations back?
