Two methods

1) Go to: Prefs > Plugins > Input
For MP3, double click MPEG Audio Decoder
Go to: Decoder tab, and uncheck "allow stereo output"

If playing a different format, look in the relevant input plugin config,
eg. for .ogg files:
Vorbis Decoder > config > Multichannel tab:
Checkmark: Downmix all to mono

etc

2) For global setting, go to:
Prefs > Plugins > DSP/Effect
Select: Nullsoft Signal Processing Studio
In the SPS window, click "Load" button
Select: mix to mono.sps
Click "Open"
Back in the SPS window, checkmark "enable processing".

If you installed the FULL version of Winamp, you can use Justin's preset:

Open Preferences > Plug-ins > DSP/Effect.
Select 'Nullsoft Signal Processing Studio' > press Configure button.
Press Load > select 'justin - mix to mono.sps > press Open.
Check 'Enable processing'.

too slow

If you're bored go here or, if the boredom is more serious, here.
