AFAIK you can add a menuitem in the Playlist-Editor by subclassing its window and add the menu on the WM_INITMENUPOPUP-message (that's how my plugin does it - but it's only copy&pasted from some code I found in here).

I don't know how to add a menuitem to a ML-menu, but I think it has to be quite similar - but maybe only ML-plugins are allowed to do this...
