Most common cause would be that your host does in fact not actually have enough bandwidth. The other possibility (albeit rare) could be that the computer that the source is running from has too much being done on it. AKA, you are at 98% resources all the time and the other 2% are being used by your source program. This can also occur if the hard drive the files are on isn't fast enough or is being accessed by other programs constantly.
