How about "File" -> "New Project" -> "ActiveX dll"? And another usefull hint: you might want to look at the examples on that page. The GenWrapper even includes one which is quite usefull to a programmer, imho...

ReadMe.txt
This contains the Visual Basic project files for VbHotKey.dll.
This is a COM (~ActiveX) Server Dynamic Link Library that
contains one class, called Plugin. This Plugin class implements
an interface called IRjlWinAmpGenPlugin. This means that when
an instance (ie a new copy) of this class is created, it can be
treated like a IRjlWinAmpGenPlugin, so that the GenWrapper dll
knows how to communicate with it. Implementing the
IRjlWinAmpGenPlugin interface is the basis of this plugin
architecture.
