Try Rovastar‘s suggestion to flush the plug-in cache. Ctrl+P > Plug-ins > click ‘Flush Plug-in Cache’ button.
Have you tried uninstalling and reinstalling R4? Also, it is a good idea to install plug-ins while Winamp is not running. Maybe you have corrupted downloads of R4. Try downloading again. If you have a third party download manager/accelerator temporarily disable it prior to downloading. Download accelerators can sometimes cause more trouble that they are worth.
If Winamp is recognizing other vis plug-ins and not R4, then I don’t think it a problem specifically related to Winamp. Once again good luck.
